About this protocol

BEDROCK is a falls-prevention programme that also builds bone, for patients on denosumab/Jubbonti, post-menopausal women, long-term glucocorticoid users, and those with prior fragility fracture. Ten core exercises — balance and proprioception first, then functional strength, then bone-loading impact — pair with Interval Walking Training on alternate days, every exercise tiered by FRAX risk category (GREEN <10%, AMBER 10–20%, RED >20%). Note the step target intentionally caps near 4,000/day with emphasis on brisk-cadence minutes, not total volume. Designed for the denosumab biosimilar PSP whitespace created by Jubbonti switching mandates.

Evidence base

BEDROCK is derived from systematic review and randomized controlled trial evidence in Osteoporosis, Bone Health & Falls Prevention. Protocol parameters — phase structure, load progression, and outcome measures — align with EULAR, ACR, and ASAS exercise-therapy recommendations. Outcome tracking favours open-access PROMIS instruments where validated equivalents exist.

Outcome measures

  • FRAX score (10-year major osteoporotic fracture probability)
  • Timed Up & Go (TUG) — fall-risk tier gating
  • 30-Second Chair Stand
  • Falls history since last session
  • BMD / DXA at intake (patient-reported)
  • Daily step count (target 4,000, not 10,000)
  • Brisk-cadence minutes/week (≥50 min/wk at ≥100 spm)

PROMIS short forms are free, NIH-developed measures used in place of paid legacy instruments.
