How integration works

PSP intake handoff

Your existing enrolment workflow triggers a FlareForward referral. We accept HL7 FHIR referral messages or a simple secure CSV upload — whichever your PSP infrastructure supports.

Coordinator outreach within 48h

A Canadian-registered care coordinator contacts the patient, conducts a virtual functional assessment, and matches them to the appropriate FlareForward protocol.

Protocol delivery over 20–26 weeks

Patients complete live-virtual and asynchronous exercise sessions. Monthly ePRO check-ins adapt programme intensity. Coordinators are alerted when scores cross clinical thresholds.

Aggregate outcomes reporting

At cohort completion, you receive a structured outcomes report: PROMIS-PF, PHQ-9, and Pain VAS trajectories, adherence rates, and programme completion rates — formatted for medical affairs.

Data & compliance

All FlareForward infrastructure is operated in Canada. Patient health information never leaves Canadian jurisdiction. Partner reporting is aggregate-only: your medical affairs team receives cohort-level ePRO trajectories, not individual records.
